Verify Your COVID-19 Vaccination in 5 Easy Steps

All members of the CU Anschutz community must be fully vaccinated, or have filed an exemption, no later than September 1.

Students and employees may request medical or religious exemptions to the vaccination requirement, per the CU Anschutz COVID-19 Vaccination Policy. Please note that with either exemption request, you will be contacted by a campus representative to validate that request; until it is validated, you are not cleared to be on campus after September 1.

Faculty & Staff: Use the web-based system detailed below to fulfill this requirement. 

Students: Submit proof of vaccination directly to your school or program. If filing an exemption request, submit the exemption form to your school or program, and submit an exemption request using the vaccine verification system. Forward your confirmation email to your school or program. 

Student Employees: Submit to your school or program, and complete the process below.

Badged Affiliates & POIs: Use the web-based system detailed below to fulfill this requirement. Use CU Anschutz credentials to sign in. Contact vaccine@cuanschutz.edu with questions.

Ensure your name is listed correctly.

Select your campus affiliation. 

Employees in consolidated units should follow the prompt to select affiliation based on their positions.

Select “I want to verify vaccination information.” Enter your vaccine type and dosage date(s). 

Find this information on your COVID-19 Vaccination Record Card or in your immunization record with your medical provider. If you have received only one dose of a two-dose vaccine, or are not yet 14 days beyond your final vaccine dose, you can submit your form by choosing "Vaccine in Process" in the verification system. You will need to re-submit your form when fully vaccinated (14 days or more beyond your final dose).

Select “I want to file an exemption.” 

You may file a medical or religious exemption, and you will be contacted by a campus representative once you have entered your request. Medical exemptions require you to upload a form signed by your licensed medical provider in PDF or JPG format. Please note that the medical exemption forms were updated August 20; ensure you are using the current form and including additional clarification.
