Disconnect via email to OIT Service Desk

Send an email to the Service Desk, specify that you are requesting to disconnect an "Adobe License service" and include the following information:

• First and Last name of the license holder
• Email address of the license holder

Disconnect via OIT Service Center (Cherwell)

1. Go to the OIT Service Center portal
   
2. Click "Login" button in top right of screen and enter your CU credentials
   
3. Click "Make a Request" button in middle of screen
   
4. Scroll down to "Software/Applications" tile and select it
   
5. Select "Adobe Acrobat" tile
   
6. Select "Disconnect" tile
   
7. Enter the First Name and Last Name of the license holder, the email of the license holder, and any other helpful information

Send an email to the Service Desk, specify that you are requesting to disconnect a "Pager service" and include the following information:

• Pager number
• Name of pager holder
• Whether or not the pager is being returned physically or if it has been lost/stolen

When returning the pager physically also use the following instructions:
Take it to the pager office (ED2 North, Rm 5102-1) and leave it at the desk, with or without a note. They can also, send it via the inter-campus mail to Pager Office, Mail Stop F408.

7. In the “Description” field explicitly state that you are wanting to disconnect a service. Enter the PBN (5 digit number) and the name/email of the PBN user (if it is a departmental PBN just provide the name assigned to the PBN)
